About YOD Reflections
At Yod Reflections, we are specifically trained and experienced in Younger Onset Dementia and understand the complexity of your needs.
We acknowledge that everyone is unique, and work with you to tailor your NDIS supports in alignment with your goals and funding. We also understand the complex nature of Dementia and can connect you with a range of supports and services (mainstream, informal or community based) that are tailored to your individual needs and circumstances.
We understand that your families’ lives are dramatically changed by a Dementia Diagnosis. Many participants and their partners are still working, have young children and are also caring for elderly parents. We can assist you in adjusting to these circumstances and to support you to understand Dementia, build capacity to problem solve and know where to turn for help.
As we only provide Support Co-ordination for people with Younger Onset Dementia, there is no conflict of interest and the support available will not be influenced and will be personally tailored to meet your individual goals.
The NDIS maintain that participants have “choice and control” when choosing supports to meet their needs. As we get to know you and your story, we can assist you to live well with Dementia.
At YOD Reflections, our aim is to not just have someone come and take you for cake and coffee, but to support you to participate in activities that are stimulating and make you feel excited and alive.
We assist in liaising with your chosen service providers including, Allied Health (Occupational Therapist, Speech Therapist, Physiotherapist), Plan Managers, Social Support providers and can assist you maintaining strong relationships that ensure the support you receive is appropriate to your personal needs.
We walk beside you with consistency, understanding and experience. As your circumstances change, so do your needs and we have the understanding to ensure the supports you receive change with you. We also support you to prepare for the future and to resolve points of crisis by drawing on your support networks.
Michelle and Joanna have degrees in Counselling and Social sciences along with training in Dementia and can connect you with the supports you need while providing compassionate and empathetic support. Our newest team member Alison has extensive experience and training in Dementia, and is well connected to supports and services on the Mid North Coast. We can also connect you with Dementia Education and Carer supports like Counselling and YOD specific Support groups for additional assistance.
The NDIS require regular progress reports and we can help you to adjust your NDIS goals and ensure you have the reports required to clearly articulate your needs in your NDIS yearly reviews. In addition, we can support you to manage your Service agreements and budget to ensure funds are utilised appropriately and last the duration of your NDIS plan.
Should your circumstances change and you require assistance to explore long term housing options, we have experience assisting participants to transfer into long term accommodation options, like Supported Independent Living (SIL) or residential care (if no suitable SIL options are available).
If you enter residential aged care prior to 65 years of age, the support of the NDIS will continue and if funding is allocated for our support, we can assist you to maintain strong relationships, attending quarterly case conferences with stakeholders and ensuring your needs are met and connection to family and community remain strong.
At YOD Reflections, we are committed to providing dementia specific, person-centered quality support co-ordination to NDIS participants with a Younger Onset Dementia diagnosis. If you have Plan Managed Support Co-ordination in your current NDIS plan, we can help you. If you don’t have Plan Managed Support Co-ordination in your plan, please contact us and we can explore options and assist you in requesting the NDIS make any changes that need to be made.
Michelle has a Bachelor of Applied Social Science in Counselling and her passion for dementia care and their quality of life has led her to study a Diploma of Dementia at The University of Tasmania.
In addition Michelle holds a Diploma of Holistic Life Care and Counselling and a Certificate IV in Health and Leisure.
Michelle is committed to ongoing Professional development and continues to attend relevant workshops and conferences each year.
Michelle has worked in residential aged care facilities, a dementia day centre and as a home care worker. In addition, she has worked for two years as a Helpline Counsellor for Dementia Australia where she provided specialised information, support and understanding to health professionals, people with dementia, their families and carers.
For the last four years Michelle has had the privilege of working as a Younger Onset Dementia Support Co-ordinator with Dementia Australia, walking alongside her clients, connecting them to supports and services and assisting them to manage their NDIS plans.
